Wellness Screening and Medical Premium Discount
***IMPORTANT NOTE: The 2024 Wellness Screening period to earn medical plan premium credits for 2025 has ended. If you have questions, please contact the Benefits Center at 1-888-60-myBBU (1-888-606-9228).***
Understanding your health starts with knowing your numbers. Learn how you can protect your health, lower your health risks, and feel your best by participating in our Wellness Screening program — which can also translate to big savings on your medical plan premiums.
As an important reminder, Quest will not share your personal health data with the Company; however, the Company does receive anonymous aggregate data for reporting purposes.
Take action to reduce your 2025 medical plan premium!
If you’re enrolled in a Company medical plan, get a screening and achieve healthy results between August 1, 2024, and September 30, 2024, you can earn up to $900 in medical plan premium discounts.
If your spouse is enrolled in a Company medical plan, they can also earn up to $900 in medical plan premium discounts. Together, you can earn a total of $1,800 per year. There is no cost to you or your enrolled spouse to participate; the cost of the Wellness Screening is paid by the Company.
Attention new hires!
If you are hired between August 1, 2024, and September 30, 2024, and you enroll in a Company medical plan, you will automatically receive all medical plan premium discounts for the remainder of 2024. You will have an extended opportunity to complete your Wellness Screening to continue receiving medical plan premium discounts for 2025. You will receive a separate communication informing you of your eligibility to complete a Wellness Screening between November 14, 2024, and December 31, 2024.
Any new hire who has a start date of October 1, 2024, or later, and who enrolls in a Company medical plan will automatically receive all medical plan premium discounts for the remainder of 2024 and for all of 2025. However, to continue receiving medical plan premium discounts for 2026, new hires must complete a Wellness Screening during the fall 2025 screening period.
If you are newly enrolling a spouse in Company medical coverage
If you are newly enrolling your spouse for coverage under a Company medical plan in 2025, he or she must wait until after the Annual Enrollment period for their information to be sent to Quest before they can get a Wellness Screening.
You will receive an email from the Company confirming when your spouse’s information has been sent to Quest, after which your spouse may take action. Beginning November 14, 2024, your spouse should follow the registration instructions to schedule an appointment at a Quest PSC or receive their screening from a BCBSIL physician. You will be eligible to receive medical plan premium discounts provided your spouse’s Wellness Screening is completed by December 31, 2024.
You should start receiving earned medical plan premium discounts for your spouse in early February 2025, and the discount will be retroactively applied back to January 1, 2025, via a payroll refund.
Attention associates and spouses over age 45!
Participating in the Wellness Screening makes testing yourself for colon cancer a simple process. associates and spouses aged 45 or older will see the option to order a Colorectal Cancer Screening home test kit when registering for a Wellness Screening through the Quest website.
If you’re not sure whether the colorectal cancer screening is right for you, here are some important facts:
- Colorectal cancer is the third most common type of cancer in the United States.
- When diagnosed early, survival rates can be as high as 90%.
- Regular screening (testing) is one of the most powerful ways to prevent colorectal cancer.
- According to the American Cancer Society, it typically takes 10-15 years for abnormal cells to develop into colorectal cancer. Regular screenings, like this, can find these cells, which then can be removed before they have a chance to grow and turn into cancer.
New! Prostate Specific Antigen (PSA) Test
Men age 45 or older can opt to receive this additional test when they schedule their Wellness Screening through the Quest website. It will be included as part of the blood-draw. This test is not used to determine eligibility for medical plan premium discounts, it is only for informational purposes.
If you're not sure whether the Prostate Specific Antigen test is right for you, here are some important facts:
- Prostate cancer is among the most common cancers affecting men, with incidence increasing with age.
- PSA tests are used for early detection and to determine who should receive biopsies of their prostate.
- PSA testing measures levels of a protein produced in the prostate. PSA levels may be elevated due to an infection of the prostate, an increase in the size of the prostate (which is common as men age), or prostate cancer.
- The chance of having prostate cancer increases as PSA levels increase, but there is no set cutoff point that can diagnose prostate cancer without a biopsy.
How to Earn Your Medical Plan Premium Discount
Here are the ways you and your enrolled spouse can receive discounts on your medical plan premiums in 2025. The Wellness Screening window is typically in August/September of each year. If you participate and receive healthy results in all three categories, you could earn the full $900 discount:
How to register for Quest
New users must visit the Quest website and create an account using the Registration Key “Bimbo” and click “Register Now” to get started. You will be prompted to enter the following information:
- First field (Login ID): Last six digits of your Social Security number and your full date of birth (SSSSSSMMDDYYYY); 14 digits total
- Second field: Your full date of birth (MMDDYYYY)
How to Get Your Wellness Screening
Option 1: Get a screening onsite or at a Quest PSC
Visit the Quest website beginning August 1 to register for a Wellness Screening using your username and password. Follow the prompts to schedule your screening and be sure to bring a printed confirmation statement to your appointment.
Visit the Quest website for more details about the Wellness Screening process and call Quest at 1-855-623-9355 if you have questions or difficulty logging in. You can also review the Wellness Screening Overview.
Here’s where you can get an onsite Wellness Screening:
To register for an onsite Wellness Screening, visit the Quest website. Remember, you must register at least two weeks in advance! Onsite screenings will be held at the following locations:
- The 2024 onsite Wellness Screenings for the 2025 medical plan discounts have ended.
Option 2: Get a screening from a BCBSIL physician
Download and print the Physician Results Form, available when you log into your account through the Quest website. New users must create an account using the Registration Key “Bimbo” and click “Register Now” to get started.
Take the Physician Results Form to your personal physician to complete during a visit. This is the only physician form that will be accepted to be eligible for discounts. associates can upload their completed Physician Results Form through their online Quest account for processing no later than September 30, 2024.

What if a Wellness Screening is not medically appropriate?
We understand that, in some instances, you may not be able to participate in a venipuncture blood test or meet one or more of the Healthy Results due to circumstances beyond your control. If it is NOT medically appropriate for you to achieve one or more of the above Healthy Results or Alternatives, please share the Healthy Standard Appeals Form with your physician to receive an appropriate Physician Alternative for the applicable health measure(s).
If your physician confirms it is not medically appropriate for you to obtain a venipuncture blood test or achieve healthy results by the screening deadline, the Company will waive the blood test requirement to earn the Wellness Screening discount.
What if you need to be reimbursed for a Wellness Screening?
We are committed to helping you understand and improve your health, which is why we encourage you to receive an annual Wellness Screening. If you are not covered under a Company medical plan when you visit your physician and receive your Wellness Screening, you can submit a Wellness Screening Reimbursement Form.
PREPARE FOR YOUR SCREENING CHECKLIST
- Do not eat or drink anything except water for nine hours before your appointment.
- Take all medications as directed by your physician.
- Avoid physical activity 30 minutes before and after your appointment.
- After your screening, eat or drink something with some sugar, such as a piece of fruit.

Preventive Care Checklists
Getting timely and relevant preventive care is an essential step in identifying health issues and chronic conditions before they become more serious. Annual physicals, age- and gender-appropriate cancer screenings and vaccinations can all help make sure you stay healthy. That’s why preventive care received from an in-network provider is always covered 100% by your Company medical plan.

Chronic Condition and Complex Medical Diagnosis Support
If you and/or your spouse has diabetes, you may join Livongo by Teladoc Health’s Diabetes Program. You will receive a glucose meter, supplies and unlimited test strips at no cost to you.
Additionally, if you and/or your spouse are currently using high blood pressure medicine, you are eligible for Livongo by Teladoc Health’s Hypertension Program to help manage your condition. Participants will receive a wireless-connected blood pressure cuff and access to a nurse who will guide participants through their readings and act as a wellness resource.

Need personalized support? Try a BCBSIL Personal Health Clinician
If you are currently living with a medical condition, such as chronic obstructive pulmonary disease (COPD) or asthma, a BCBSIL Personal Health Clinician can help you live better and avoid complications. Based on health care claims data, you and/or your enrolled spouse may receive a call from a BCBSIL Personal Health Clinician — experienced registered nurses, pharmacists, dietitians, and professionals trained in psychology and social work.
We encourage you to take the call or contact a BCBSIL Care Manager at 1-877-239-7449 (Customer Service).

Addiction Support
The Company is committed to offering programs that create a healthy and productive work environment. For this reason, the Company has enhanced the tobacco cessation program to include support on cutting back on alcohol and overcoming opioid dependence.
Pelago is a confidential program to help people quit smoking, cut back on alcohol*, or overcome opioid dependence* with the help of scientifically proven techniques and expert coaches and counselors, right from your mobile device. The program is tailored to your personal journey.

Please note:
If you are enrolled in a Company medical plan, and your 2023 Wellness Screening results show you or your enrolled spouse are not tobacco-free, you can retroactively earn the tobacco-free medical plan premium discount when you or your spouse complete the Pelago tobacco cessation program by July 31, 2024.
Employee Assistance Program (EAP)
SupportLinc is the Company’s Employee Assistance Program (EAP). It is a confidential resource that helps you deal with life’s challenges and the demands that come with balancing home and work. Through SupportLinc, you can access a variety of services, including professional counseling, expert referrals for a wide array of personal and work-related concerns, information on health and wellness and more.
The use of SupportLinc’s services is completely confidential and provided at no cost to all Company associates and their family members. Representatives are available 24/7 via phone, video, text, in-person or online, so reach out for help whenever you may need it.

Telehealth for Mental Health Support
You can also access virtual mental health services through MDLIVE for issues such as anxiety, depression, child behavioral issues and marital problems. These visits are a convenient alternative to a traditional visit to a doctor’s office.

Flu Shot
All associates and eligible dependents can receive a free annual flu shot. Getting a flu shot not only helps lower your chances of catching the flu, but also helps reduce the severity of your symptoms and can keep you out of the hospital if you do come down with it.
If you enroll in a Company medical plan, visit a BCBSIL in-network health care provider or pharmacy and show your BCBSIL card to receive your free annual flu shot. Your enrolled dependents can get a free flu shot too!
Flu shots can be processed under a Company prescription card at a participating pharmacy of your choice. The cost is covered 100% for associates and their dependents who are enrolled in a Company medical plan when they present their medical card to the pharmacist—no out-of-pocket costs necessary.
If you are not enrolled in a Company medical plan, vouchers are available for associates and immediate family members age 18 and older. Visit the Quest website for more information.
Blue365 Discount Program
Associates enrolled in a Company medical plan have access to Blue365through BCBSIL*. With the Blue365 program, you and your family can save money on health care products — and services — that typically aren’t covered by your medical plan. Some discounts include:

- Fitness gear and apparel
- Family activities
- Healthy eating options
There are no claims to file, and no referrals or prior authorizations needed. All you need to do is register on the Blue365 website and enter your BCBSIL insurance card information.
