Wellness Screening and Medical Plan Premium Discounts
ATTENTION: Each year the Company invites you and your spouse to complete a Wellness Screening to earn discounts on medical plan premiums — up to $900 per person ($1,800 in total).
NOTE FOR ANNUAL ENROLLMENT: If you or your spouse are considering newly enrolling in a Company medical plan during this year’s Annual Enrollment (for benefits effective January 1, 2027), you must complete the 2nd Round Screening (January 1–January 31, 2027), to be eligible for 2027 medical plan premium discounts.
As an important reminder, Quest will not share your personal health data with the Company; however, the Company does receive anonymous aggregate data for reporting purposes.
Important Eligibility Requirements
If you are hired or have a qualified life event during this period:
- August 1–December 1, 2026:
- To earn 2027 medical plan premium discounts: You and your spouse must complete the 2nd Round Screening (January 1–January 31, 2027). This includes associates and spouses newly added during Annual Enrollment.
- On or after December 2, 2026:
You will receive information in 2027 regarding the Wellness Screening program.
Attention associates and spouses over age 45!
Participating in the Wellness Screening makes testing yourself for colon cancer a simple process. associates and spouses aged 45 or older will see the option to order a Colorectal Cancer Screening home test kit when registering for a Wellness Screening through the Quest website. You can order your kit anytime between August 1, 2026, and January 31, 2027, and must return your kit by March 31, 2027.
If you’re not sure whether the colorectal cancer screening is right for you, here are some important facts:
- Colorectal cancer is the third most common type of cancer in the United States.
- When diagnosed early, survival rates can be as high as 90%.
- Regular screening (testing) is one of the most powerful ways to prevent colorectal cancer.
- According to the American Cancer Society, it typically takes 10-15 years for abnormal cells to develop into colorectal cancer. Regular screenings, like this, can find these cells, which then can be removed before they have a chance to grow and turn into cancer.
Prostate Specific Antigen (PSA) Test (Only available through a Quest PSC)
If you are age 45 or older and have a prostate, you can opt to receive this additional test when they schedule their Wellness Screening at a Quest PSC through the Quest website. It will be included as part of the blood-draw. This test is not used to determine eligibility for medical plan premium discounts, it is only for informational purposes.
If you're not sure whether the Prostate Specific Antigen test is right for you, here are some important facts:
- Prostate cancer is among the most common cancers affecting men, with incidence increasing with age.
- PSA tests are used for early detection and to determine who should receive biopsies of their prostate.
- PSA testing measures levels of a protein produced in the prostate. PSA levels may be elevated due to an infection of the prostate, an increase in the size of the prostate (which is common as men age), or prostate cancer.
- The chance of having prostate cancer increases as PSA levels increase, but there is no set cutoff point that can diagnose prostate cancer without a biopsy.
How to Earn Your Medical Plan Premium Discount
Here are the ways you and your enrolled spouse can receive discounts on your medical plan premiums. If you complete a Wellness Screening and receive healthy results in all three categories, you could each earn the full $900 discount:
How to register for Quest
New users must visit the Quest website and create an account using the Registration Key “Bimbo” and click “Register Now” to get started. You will be prompted to enter the following information:
- First field (Login ID): Last six digits of your Social Security number and your full date of birth (SSSSSSMMDDYYYY); 14 digits total
- Second field: Your full date of birth (MMDDYYYY)
Please note: Spouses will have to register separately with their own user name and password.
How to Get Your Wellness Screening
Option 1: Get a screening onsite or at a Quest PSC
Visit the Quest website beginning August 1 to register for a Wellness Screening using your username and password. Follow the prompts to schedule your screening and be sure to bring a printed confirmation statement to your appointment.

- Zanesville, OH (Bimbo QSR Airport) Bakery
- Zanesville, OH (Bimbo QSR E Pointe) Bakery
To register for an onsite Wellness Screening during the Wellness Screening period, visit the Quest website. Remember, you must register at least two weeks in advance!
Option 2: Get a screening from a BCBSIL physician
Download and print the Physician Results Form, available when you log into your account through the Quest website. New users must create an account using the Registration Key “Bimbo” and click “Register Now” to get started.
Take the Physician Results Form to your personal physician to complete during a visit. This is the only physician form that will be accepted to be eligible for discounts. Associates can upload their completed Physician Results Form through their online Quest account for processing no later than September 30, 2026, for the 1st Round Screening (August 1–September 30, 2026) for 2027 medical plan premium discounts and no later than January 31, 2027, for the 2nd Round Screening (January 1–January 31, 2027) for 2027 medical plan premium discounts.

What if a Wellness Screening is not medically appropriate?
We understand that, in some instances, you may not be able to participate in a venipuncture blood test or meet one or more of the Healthy Results due to circumstances beyond your control. If it is NOT medically appropriate for you to achieve one or more of the above Healthy Results or Alternatives, please share the Healthy Standard Appeals Form with your physician to receive an appropriate Physician Alternative for the applicable health measure(s).
If your physician confirms it is not medically appropriate for you to obtain a venipuncture blood test or achieve healthy results by the screening deadline, the Company will waive the blood test requirement to earn medical plan premium discounts.
What if you need to be reimbursed for a Wellness Screening?
We are committed to helping you understand and improve your health, which is why we encourage you to receive an annual Wellness Screening. If you are not covered under a Company medical plan when you visit your physician and complete your Wellness Screening, you can submit a Wellness Screening Reimbursement Form.
PREPARE FOR YOUR SCREENING CHECKLIST
- Do not eat or drink anything except water for nine hours before your appointment.
- Take all medications as directed by your physician.
- Avoid physical activity 30 minutes before and after your appointment.
- After your screening, eat or drink something with some sugar, such as a piece of fruit.
